Approve retroactive Agreement with Redwood Quality Management Company for up to $120,000 to provide support, implementation, and participant-specific data to the County of Mendocino’s Whole Person Care Pilot Project for the term of July 1, 2017 through June 30, 2018; authorize the Health and Human Services Agency Director or designee to sign any future amendments to the Agreement, including additional funding up to $400,000 in order to complete grant activities in FY 2017-18; and authorize Chair to sign same.

Summary of Request

The Whole Person Care (WPC) Pilot Program is a significant, new, grant-funded project with substantial needs for community participation. The goal of the WPC program is to provide comprehensive coordinated care resulting in better health outcomes among participants with Severe Mental Illness.

 

Redwood Quality Management Company (RQMC) is one of several community partners identified and authorized during the grant application process. Under the guidelines specified in the proposed Agreement, RQMC will participate in all relevant activities related to the implementation of the WPC Pilot Program. These activities include: developing infrastructure and providing staff for a Healthy Living Center in Fort Bragg; participating in WPC Steering Committee meetings; participating in biweekly Adult Multidisciplinary Teams that focus on care coordination of WPC participants; participating in Ad-Hoc Data Workgroup meetings; and providing data for identified groups of patients that is compliant with Healthcare Effectiveness Data and Information Set guidelines.

 

Funding for this Agreement is provided by the federal Department of Health and Human Services Centers for Medicare and Medicaid Services through the California Department of Health Care Services. The funder notified the County in late June 2017 of the award. Final directions about invoicing and reporting were not issued by the funder until mid-October, and were required to be incorporated into the subcontract language. These delays impacted the County’s ability to complete the contract before the start date of the project.
