Approve amendment to Agreement No. MH-17-008, PA No. 18-11 with Telecare Corporation increasing the amount from $50,000 to $93,000 to provide services as a skilled nursing facility for clients of Health and Human Services Agency, Behavioral Health and Recovery Services beginning upon execution by all parties and continuing through fiscal year 2017-18; authorize the Health and Human Services Agency Director or designee to sign any future amendments that do not increase the annual maximum amount; and authorize Chair to sign same.

SUMMARY OF REQUEST:
Garfield Neurobehavioral Center is a Skilled Nursing Facility operated by Telecare Corporation in Alameda County. Garfield Neurobehavioral Center provides safe, comprehensive medical, nursing, and rehabilitative services for adults who have both a mental illness and a behavioral impairment associated with brain disease (e.g., disorders related to head injury, tumors, stroke, or dementia; other disorders associated with drug or alcohol use; or disorders related to progressive conditions such as multiple sclerosis or Parkinson's). The proposed amendment provides additional funding for services rendered to conserved individuals through Mendocino County's Continuum of Care.
