Approve amendment to BOS Agreement No. 17-084 with Crestwood Behavioral Health, Inc., increasing the amount from $260,000 to $428,449 to provide residential treatment services for Health and Human Services Agency Behavioral Health and Recovery Services clients term beginning upon execution by all parties and continuing through FY 17-18; authorize the Health and Human Services Agency Director or designee to sign any future amendments to the Agreement that do not increase the annual maximum amount; and authorize Chair to sign same.

SUMMARY OF REQUEST:
Crestwood Behavioral Health, Inc., provides secure residential treatment programs that deliver a broad range of services for adults requiring a structured environment due to mental health challenges. Levels of care include the provision of groups, trainings, and programs to promote the recovery of clients for independent living in the community.
