Approve retroactive Agreement with Redwood Quality Management Company in the amount of $1,555,000 to provide direct service, facilitation, administration and participant-specific data to the County of Mendocino's Whole Person Care Pilot Project for the period of January 1, 2018 through December 31, 2018; authorize Health and Human Services Agency Director or designee to sign future amendments that do not increase the annual maximum amount; and authorize Chair to sign same.

SUMMARY OF REQUEST:
The Whole Person Care Pilot Program is a significant, new, grant-funded project that provides comprehensive coordinated care resulting in better health outcomes among participants with significant mental health conditions. Redwood Quality Management Company is one of several community partners identified and authorized during the grant application process. Under the guidelines specified in this Agreement, the contractor will provide: comprehensive coordination of care, wellness coaches, mental health resource centers, mental health transitional support and connections coordinators.
