Approve fourth amendment to BOS Agreement No. 17-075 with Ford Street Project in the amount of $20,000, for a total Agreement amount of $80,000, to provide standard 5 day detoxification treatment services for clients referred by the Probation Department; extend the termination date from June 30, 2018 to June 30, 2019; and authorize Chair to sign same.

PREVIOUS BOARD/BOARD COMMITTEE ACTIONS:
The Board of Supervisors approved the third amendment of the original Department Agreement on August 1, 2017, increasing the agreement by $20,000, for a total agreement amount of $60,000 and extending the termination date from June 30, 2017 to June 30, 2018.

SUMMARY OF REQUEST:
Ford Street Project provides, on a twenty-four hour basis, a licensed social model detox program for clients referred by the Probation Department. Services include: transportation from Ford Street and oversight of prospective client to appropriate medical provider to obtain medical clearance for social detox, the provision of short term emergency shelter, referral to health services including outpatient detox, general alcohol education, food services, screening for residential treatment referral, short term crisis intervention counseling, emergency clothing, and personal hygiene support and supplies.
