Approve Agreement with Telecare Corporation in the Amount of $150,380 to provide medical, nursing, and rehabilitative services to individuals with neurological and mental deficits for the term of July 1, 2018 through June 30, 2019; authorize the Health and Human Services Agency Director or designee to sign any future amendments to the Agreement that do not increase the annual maximum amount; and authorize Chair to sign same.

SUMMARY OF REQUEST:
Telecare Corporation operates a Skilled Nursing Facility in Alameda County that provides safe, comprehensive medical, nursing, and rehabilitative services for adults who have both a mental illness and a behavioral impairment associated with brain disease (e.g., disorders related to head injury, tumors, stroke, or dementia; other disorders associated with drug or alcohol use; or disorders related to progressive conditions such as multiple sclerosis or Parkinson's). Telecare's treatment approach is based on optimizing each individual's potential to reach his/her highest level of functioning. Daily resident activities focus on: minimizing the negative impact of each resident's neurological disorder; skill building as it relates to daily living; medical evaluation and monitoring; and working on individual goals.
