Adopt Resolution approving Department of Transportation (DOT) Agreement Number (No.) 180018, Professional Services Agreement with Stantec Consulting Service, Inc., (Stantec) in the amount of $172,720.46 for Vegetation Monitoring and Maintenance Services for the term starting upon execution of the agreement through January 31, 2023, and authorizing an additional contingency amount of $17,000 for the Forsythe Creek Bridge, on Reeves Canyon Road, County Road (CR) 219, milepost (MP) 0.46 (Redwood Valley); and authorize Chair to sign same.

Previous Board/Board Committee Actions:
By Resolution No. 12-003 (January 10, 2012) the Mendocino County Board of Supervisors (BOS) approved DOT Agreement No. 110103, Professional Services Agreement with Drake Haglan and Associates, for the replacement of the Forsythe Creek Bridge on Reeves Canyon Road.
By Resolution No. 14-136 (October 7, 2014) the BOS adopted an Initial Study/Mitigated Negative Declaration and a Mitigation Monitoring and Reporting Program in compliance with the California Environmental Quality Act requirements for the replacement of bridge No. 10C0077 on Reeves Canyon Road, CR 219, M.P. 0.46 over Forsythe Creek.
By Resolution No.17-072 (June 6,2017) the BOS approved amendment No. 7 to the BOS Agreement No. 12-006/DOT Agreement No. 110103, for Construction Design Support Services with Drake Haglan and Associates for a total amount of $57,561 for the design of the Forsythe Creek Bridge replacement on Reeves Canyon Road.
By Resolution No. 18-033 (February 27, 2018) the BOS approved Notice of Completion and Release of Contract Surety, DOT Contract No. 160022, for the Forsythe Creek Bridge replacement on Reeves Canyon Road.
Summary of Request:
DOT Agreement No. 180018 provides for vegetation replacement, erosion control measures, monitoring, and maintenance for five years. Stantec and their sub-consultant, Sequoia Ecological Consulting, Inc., responded to the request for proposals and were rated most qualified to do the work. The negotiated “not-to-exceed” price is $172,0720.46 with a contingency of $17,000 for additional tasks that may arise during the five years.
