Approve amendment to Agreement No. PH-17-038, PA 18-90, with Patricia Humphrey Warner, DVM, increasing the amount from $50,000 to $95,000 to provide veterinary services for Animal Care Services for the period of July 1, 2017, through June 30, 2019; authorize the Chief Executive Officer or designee to sign any future amendments to the Agreement that do not increase the annual maximum amount; and authorize Chair to sign same.

SUMMARY OF REQUEST:
Patricia Humphrey Warner, DVM has been providing veterinary services at the Mendocino County Animal Shelter since September 1, 2016. These services include: routine health exams; vaccinations for dogs and cats; surgical procedures with pre-exams and post-instructions; professional opinion on injuries; and testimonies in court or administrative hearings, as needed. Dr. Warner is a skilled veterinarian willing to work additional hours at the Shelter to perform these services as well as take on operational duties required to run the Animal Shelter.
