Approve the retroactive Agreement with Adventist Health, Ukiah Valley in the amount of $157,800 to provide support and participant-specific data to the County of Mendocino’s Whole Person Care Pilot Project for the period of January 1, 2018, through December 31, 2018; authorize the Health and Human Services Agency Director or designee to sign any future amendments that do not increase the annual maximum amount; and authorize Chair to sign same.

Summary of Request:
The Whole Person Care Pilot Program (WPC) is a significant, new, grant-funded project that provides comprehensive coordinated care resulting in better health outcomes among participants with significant mental health conditions. Adventist Health, Ukiah Valley is one of several community partners identified and authorized during the grant application process to participate in the project. Under the guidelines specified in this Agreement, the Adventist Health will participate in the following: WPC Steering Committee meetings to design and evaluate the work case flow; Ad-Hoc Data Workgroup meetings to synthesize and streamline the different data collection methods used; and Adult Multidisciplinary Team meetings to focus on the care coordination of WPC participants. Adventist Health will also provide data regarding Emergency Room visits and hospitalizations of WPC participants.
In February 2018, during the Board’s Public Health, Safety and Resources Committee meeting, Health and Human Services Agency delivered an informational presentation regarding the Whole Person Care Pilot Program. This presentation explained that the grant’s funding runs through the calendar year and that Health and Human Services Agency received approval from the State for budget adjustments and rollover after the beginning of the grant year in regards to this program.
