Approve Agreement with North Coast Opportunities, Inc. in the amount of $150,000 to hire and supervise staff in support of the Healthy Mendocino Community Health Improvement Plan, effective when Agreement becomes fully executed by all parties through October 31, 2019; authorize the Health and Human Services Agency Director or designee to sign any future amendments to the Agreement that do not increase the annual maximum amount; and authorize Chair to sign same.

SUMMARY OF REQUEST:
In 2011, the Health and Human Services Agency Advisory Board launched the Healthy Mendocino initiative with the goal of improving quality of life and policy decision making in Mendocino County through the sharing of accurate local data. With support from a coalition of community partners, Healthy Mendocino sponsors projects that encourage informed dialogue about the actions local residents and organizations can take to improve community health. One such project is the Community Health Improvement Plan (CHIP). CHIP, which builds on the Community Health Needs Assessment completed in early 2016, engages citizens and organizations within Mendocino County to improve local health and the factors that influence it.
