RECOMMENDED ACTION/MOTION:

Approve retroactive Agreement with Manzanita Services, Inc. in the amount of $260,000 to provide housing support services for individuals with Serious Mental Illness at the Wellness Centers in Ukiah and Willits, effective July 1, 2019 through June 30, 2021; authorize the Health and Human Services Agency Director or designee to sign any amendments to the Agreement that do not increase the maximum amount; and authorize Chair to sign same.

SUMMARY OF REQUEST:
Manzanita Services, Inc. is the only agency in inland Mendocino County operating Mental Health Wellness Centers that provide specialized services for individuals with Serious Mental Illness (SMI). Through the proposed contract, Manzanita will work closely with other local agencies, including the Building Bridges program operated by Redwood Community Services, to engage individuals who have barriers to treatment for their SMI. These services include housing support services, such as rental assistance and housing relocation and stabilization services for individuals experiencing, or at risk of experiencing, homelessness, and additional supports, such as education, counseling, and linkage to treatment services to support ongoing engagement in healthcare.
