Summary of Request

On June 20, 2017, the Mendocino County Board of Supervisors (BOS) approved Mendocino County Health and Human Services Agency’s (HHSA) application for the Whole Person Care (WPC) Pilot Project, and executed the resulting revenue agreement with the California Department of Health Care Services (DHCS). WPC funding from the agreement with DHCS covers five fiscal years, 2016-17 through 2020-21, with funding ending in December 2020, under the State’s Medi-Cal Waiver. WPC funding is for prioritization of services for persons with Serious Mental Illness (SMI) and additional barriers including homelessness or housing instability. The goal of the WPC program is to provide comprehensive coordinated care resulting in better health outcomes among participants with SMI.

 

Success of the WPC program depends on the collaborative participation of many community organizations. HHSA identified several community partners for the WPC Pilot Project during the WPC grant application process including Redwood Quality Management Company (RQMC). RQMC is a required partner for the WPC project due to their key role as the County Administrative Services Organization to coordinate care for individuals with SMI. Through BOS Agreement No. 18-170, as approved by the BOS on December 18, 2018, RQMC participates in all activities relevant to the continued implementation of the WPC program including, but not limited to, attending WPC Steering Committee meetings to design and evaluate work case flow; participating in Care Coordination/Data Sharing; and providing Healthcare Effectiveness Data and Information Set compliant data for WPC participants.

 

This Amendment augments $202,000 of WPC funds specifically and only for the Mental Health Transitional Support service in 2019, and adds $1,315,300 for an additional year of services in 2020. The Mental Health Transitional Support services provided in 2019 are for temporary housing for WPC enrollees in need of safe and stable housing while they work toward their wellness goals. Services provided in 2020 are very similar to services provided in 2019 with the addition of scheduling, coordinating, and facilitating Complex Case Conferences and Client Case Conferences; supervising and training Wellness Coaches who will be housed dually at RQMC and Specialty Mental Health Service Provider sites, providing services for WPC enrollees; increasing project reporting overall, including WPC services provided by Specialty Mental Health Service providers; Medical Respite services; increasing oversight and accountability for the WPC project; and additional County responsibilities.

 

This Amendment was initially drafted in October 2019, immediately after DHCS approved the additional $202,000 allocation through approval of a Rollover Request in April of 2019 and a Budget Adjustment request in October of 2019. Both of these approvals from DHCS took place during the 2019 Program Year and after the contract with RQMC had begun. To provide the BOS with the most clear and concise picture of WPC funding for this Agreement, the program has chosen to request approval for a single retroactive amendment that incorporates the additional calendar year 2019 funds and the new amount for calendar year 2020. Corresponding Scopes of Work have likewise been updated for each year.
