Approve retroactive Agreement with Applied Survey Research, in the amount of $49,700, for evaluation and consultation services related to the California Home Visiting Program grant, effective February 1, 2022 through June 30, 2023; authorize the Public Health Director or designee to sign any future amendments to the agreement that do not increase the annual maximum amount; and authorize Chair to sign same.

SUMMARY OF REQUEST:
On November 16, 2021, Mendocino County Public Health (PH) released Request for Proposal (RFP) No. 041-21, for a California Home Visiting Program (CHVP) Evaluator. On January 3, 2022, PH awarded the RFP to Applied Survey Research (ASR), a consulting firm located in Rocklin, California specializing in social research, and providing expertise and insights to help cities, counties, and organizations measure and advance the quality of life for the people and communities served.

After the RFP award, PH and ASR began negotiating the proposed retroactive Agreement for evaluation and data collection, implementation guidance and analysis, and reporting, as required by the CHVP Grant.
