Ratify submission of grant application to Partnership HealthPlan of California (PHC), for the PHC California Advancing and Innovating Medi-Cal grant program; approve resulting grant Agreement with PHC in the amount of $339,106, for services to improve the health and care for Medi-Cal members enrolled with PHC, effective upon signing through December 31, 2023; authorize the Director of Behavioral Health and Recovery Services or designee to sign any amendments that do not increase the maximum amount; and authorize Chair to sign same.

SUMMARY OF REQUEST:
The Department of Health Care Services (DHCS), California Advancing and Innovating Medi-Cal's (CalAIM) Enhanced Care Management (ECM) and Implementation of Optional Community Supports (ILOS) programs, launched January 1, 2022. These programs require significant new investments in care management capabilities, ECM and ILOS infrastructure, information technology (IT) and data exchange, and workforce capacity across Managed Care Plans (MCPs), city and county agencies, providers, and other community-based organizations. In order to support and expand upon ECM and ILOS programs, DHCS has implemented the CalAIM Incentive Payment Program (IPP).
