Adopt Resolution ratifying submission of application for Whole Person Care Pilot Program, Second Round, with the California Department of Health Care Services in the amount of $10,504,720 for FY 2016-17 through 2020-21; authorize the Health and Human Services Agency Director or designee to sign any amendments to the Agreement that do not increase the maximum total amount; and authorize Chair to sign same.

SUMMARY OF REQUEST:
Whole Person Care (WPC) is a program within California's Section 1115(a) Medicaid Waiver Renewal, entitled Medi-Cal 2020, which was approved by the Centers for Medicare and Medicaid Services (CMS) on December 30, 2015. WPC Pilots were designed to provide support to integrate care for a particularly vulnerable group of Medi-Cal beneficiaries who have been identified as high users of multiple systems and continue to have poor health outcomes. WPC Pilots focus on the coordination of health, behavioral health, and social services, as applicable, in a patient-centered manner with the goals of improved beneficiary health and well-being through more efficient and effective use of resources.
