Adopt Resolution approving Amendment no. 4 to Board of Supervisors Agreement no. 12-034/Department of Transportation Agreement no. 110118, Professional Services Agreement with GHD, Inc., in the amount of $157,727 for the replacement of the Mill Creek Bridge on Hill Road, County Road 327B (Covelo area); and authorize Chair to sign same.

PREVIOUS BOARD/BOARD COMMITTEE ACTIONS:
By Resolution No. 12-054 (March 27, 2012) Board approved BOS Agreement No. 12-034/DOT Agreement No. 110118 with GHD Inc., for the replacement of the Mill Creek Bridge on Hill Road. By Resolution No. 15-092 (June 16, 2015) Board approved Amendment No. 3 for additional soil sampling and footing design.
SUMMARY OF REQUEST:
The California Department of Transportation has required some additional analysis and design changes that caused revisions to numerous environmental documents, right-of-way acquisition areas, and additional staff time to make the revisions. GHD has agreed to perform this work for an additional $157,727.
ALTERNATIVE ACTION/MOTION:
Postpone the approval of Amendment No. 4.
