Approve fourth amendment to Revenue Agreement 14-90070 with California Department of Health Care Services Substance Use Disorder Services to provide SUD services for FY 2014-17; adopt Resolution authorizing the Health and Human Services Agency Assistant Director/Behavioral Health Director to sign Revenue Agreement 14-90070 A04 and any future amendments to the Agreement that do not affect the annual maximum amount; and authorize Chair to sign same.

SUMMARY OF REQUEST:
The California Department of Health Care Services (DHCS) entered into a multi-year revenue Agreement with the County of Mendocino (Agreement 14-90070) for the purpose of providing local Substance Use Disorder (SUD) services, including alcohol and drug services, for FY 2014-2017. This fourth amendment put forth by DHCS modifies terms and conditions, and adjusts the 2016 SAPT Block Grant and State General Fund Awards. The resulting multi-year revenue total is $3,224,937.
