Approve Agreement with ARCH in the amount of $69,772, for services that assist community health center patients in maintaining their enrollment in Medi-Cal in fiscal years 2016-17 and 2017-18; authorize the Health and Human Services Agency Director to sign any future amendments to the Agreement that do not increase the annual maximum amount; and authorize Chair to sign same.

SUMMARY OF REQUEST:
With this Agreement, the Mendocino County Health and Human Services Agency aims to improve the percentage of community health center patients who successfully complete the renewal process for Medi-Cal coverage. The project is intended to reduce attrition rates for our Medi-Cal population, thereby ensuring timely access to primary health care needs and thus improve health. The Department of Health Care Services, Medi-Cal Eligibility Division, has provided this funding to counties through the implementation of SB18 to provide Medi-Cal outreach and renewal assistance services to our County's residents. The Agreement period is July 1, 2016 through June 30, 2018.
