RECOMMENDED ACTION/MOTION:

Ratify and approve the submission of a funding proposal to County Medical Services Program (CMSP) for a Wellness and Prevention Pilot Project to provide local-level wellness and prevention services to CMSP eligible persons in the total amount of $225,000 during fiscal years 2016-19 and authorize the Health and Human Services Agency Director to sign subsequent agreement if funding is granted.

SUMMARY OF REQUEST:
The CMSP Governing Board seeks to test the effectiveness of providing local-level wellness and prevention services to CMSP eligible persons. Projects may address any or all of the three project areas: 1) Community Wellness; 2) Whole Person Care; 3) Addressing the Social Determinants of Health over a 3-year period. Our proposal addresses Social Determinants of Health identified in the Community Health Improvement Plan (CHIP) with outcomes to include: an increase in enrollment; an increase in enrollee participation in the primary care benefit; a decrease in mental health crises addressed in hospital emergency rooms by 5%; a decrease in 5150 arrests by 5%.

This 3-year project will be accomplished in collaboration with the following Community Based Organizations (CBO): North Coast Opportunities, FIRST 5 and the Alliance for Rural Community Health. CBO expenses will pay paid by the CMSP G...
