Approve retroactive Agreement with Mendocino Coast Clinics, Inc. in the Amount of $82,500 to provide support and participant-specific data to the County of Mendocino's Whole Person Care Pilot Project for the period of January 1, 2018 through December 31, 2018; authorize the Health and Human Services Agency Director or designee to sign any future amendments to the Agreement that do not affect the annual maximum amount; and authorize Chair to sign same.

SUMMARY OF REQUEST:
The Whole Person Care Pilot Program (WPC) is a significant, new, grant-funded project that provides comprehensive coordinated care resulting in better health outcomes among participants with significant mental health conditions. Mendocino Coast Clinics, Inc. is one of several community partners identified and authorized during the grant application process. Under the guidelines specified in this Agreement, the contractor will participate in WPC Steering Committee meetings to design and evaluate the work case flow, Ad-Hoc Data Workgroup meetings to synthesize and streamline the different data collection methods used, the development of the WPC Charter and the Data Sharing Memorandum of Understanding and provide Healthcare Effectiveness Data and Information.
