RECOMMENDED ACTION/MOTION:

Approve retroactive revenue Agreement with Partnership HealthPlan of California for funds in the amount of $618,404 to support critical housing and housing-related needs of Partnership HealthPlan members through the Local Innovation Grant on Housing, effective June 17, 2019 through June 17, 2021; direct the Auditor-Controller to create a new Org Code within Budget Unit 4072 to track expenditures of grant funds; authorize the Health and Human Services Agency Director or designee to sign any renewals or amendments that do not exceed the maximum amount; and authorize Chair to sign same.

In July 2017, Partnership HealthPlan of California (PHC), the Medi-Cal Managed Care Provider for 14 California counties including Mendocino, issued a Request for Proposal (RFP) to provide one-time support for projects that would expand access to housing for Medi-Cal members enrolled with PHC.
